Copy the embed code
In WittyForm, go to your form Share settings and copy the embed snippet for your form.
Add to Shopify
In your Shopify admin, add a Custom Liquid section or HTML block to any page and paste the embed code.
Go live
Save and preview. The form renders on your storefront and submissions sync to WittyForm.

Setup Guide
In WittyForm, open your form and go to Share. Copy the embed code.
In Shopify admin, go to Online Store, then Themes, then Customize.
Add a Custom Liquid or Custom HTML section. Paste the WittyForm embed code.
Save and publish. The form appears on your storefront and responses sync to WittyForm.
