Multi-Question Fields

Multi-question fields let you group several related questions into a single visual block on your form. Instead of scattering related inputs across the page, you combine them under one label. This keeps your form organized and makes it easier for respondents to understand what information belongs together.

What Are Multi-Question Fields?

A multi-question field is a container that holds two or more sub-fields, all displayed together as one cohesive group. Each sub-field has its own input type, label, placeholder, and validation rules, but they share a single parent label and are visually grouped on the form.

Think of it like a row on a paper form where several blanks sit side by side under one heading. For example, a "Full Name" group might contain First Name, Middle Name, and Last Name sub-fields.

When to Use Multi-Question Fields

Multi-question fields work well for data that naturally belongs together:

  • Contact information: Group phone number, email, and website into one block.
  • Full name: Combine first name, middle name, and last name under a single "Name" label.
  • Address: Group street address, city, state, zip code, and country into one address block.
  • Date and time: Combine a date picker with a time picker for scheduling questions.
  • Measurements: Group a numeric value field with a unit dropdown (for example, height in feet and inches).

Creating a Multi-Question Page

A Multi-Question Page is a block that holds several questions and shows them together on one screen. You add the questions directly inside the block, so there is nothing to wire up separately.

  1. In the field panel on the left, find the Multi-Question field type under the Advanced category, then click it or drag it onto the canvas. The block starts with one question already inside it.
  2. Select the block. The right settings panel shows the questions on the page. Click Add question to add more.
  3. For each question, set its label and pick its type (short text, number, dropdown, date, rating, and so on). Use the arrows to reorder questions, or the X to remove one.
  4. Set the block label that describes the whole page (for example, "Shipping Address").

The questions you add live inside the page. They do not appear anywhere else on your form, so respondents see each question one time, grouped together.

Configuring Each Question

Every question on the page is configured on its own. In the block settings you can edit a question's label inline and change its type from the dropdown. Click Edit to open its full settings, where you can set:

  • Label: The label shown above the input (for example, "First Name").
  • Placeholder: Hint text inside the input.
  • Field type: Short text, number, email, dropdown, date, rating, and the other supported types.
  • Required: Mark a question as required on its own. Each question on the page is validated independently, so not every question has to be required.

A back link in the settings panel returns you to the page. When a respondent tries to continue past the page, every required question on it is checked, and any that is empty or invalid shows its error message right below the input.

Layout

Select the Multi-Question block to control how the included questions are arranged:

  • Vertical Stack: The questions are stacked one above the other inside the group.
  • Grid Layout: The questions are placed side by side. Choose a Columns count from 1 to 4 to set how many sit in each row. On small screens the grid collapses to a single column automatically.

Each question is shown as a card that inherits your form theme, so the group stays consistent with the rest of your form.

Data in Responses

Because every question in a group is a normal field, its answer is stored on its own, keyed by that question, exactly like a standalone question. Grouping changes only how the questions are shown on screen, not how their answers are saved.

When you export responses to CSV, each question in the group becomes its own column, so the data stays flat and easy to work with in a spreadsheet.
